Macbook Pro 13'' 8gb

So, my computer has been restarting frequently and today I finally caught it before restarting and got the panic report. I'm not really sure what it means, so any help would be appreciated. Also, it seems to shut off mostly when I move it, so I'm thinking it would be some hardware issue. Before it restarts staticky colored bars appear on the screen, and the computer freezes. I have already tried cleaning out the fan and removing and reinserting the RAM. I have a crack on my screen and the screen is a little loose on the right side.
